Purines in food: what raises uric acid, and by how much

Purines are the building blocks of DNA and RNA, so every cell contains them and every food made of cells contains some. Humans break them down to uric acid, and when uric acid is high enough for long enough it crystallises in a joint. That is gout — and the diet advice attached to it has changed a great deal in the last twenty years.

This page is not a diagnosis and not a treatment plan. Gout is diagnosed clinically, and a hot swollen joint can also be an infection, which is an emergency. Diet alone rarely brings serum urate to target: current rheumatology guidance puts urate-lowering medication first for people who need it, with dietary measures alongside. Never stop or adjust prescribed urate-lowering or anti-inflammatory medication on the basis of a food table.

Where uric acid actually comes from

Roughly two thirds of the body’s uric acid comes from ordinary cell turnover — your own DNA being recycled — and only about a third from food. That ratio is the single most useful fact on this page, because it sets a ceiling on what any diet can achieve. Beyond that, most of the variation between people is in excretion rather than production: the kidney handles urate, and how well it does so is largely genetic, which is why gout runs in families and why two people on the same diet can have very different urate levels.

Purine tables band foods as high, moderate and low, usually with the high band starting somewhere above 200 mg of purine bases per 100 g and the low band under about 100 — but the cut-offs differ between tables, and so do the values, because different laboratories measure different purine bases and different national tables use different foods. The band is the reliable part; the decimal is not.

Adenine and guanine, the purines in meat and fish, are far more urate-producing than the others, which is part of why animal purines and plant purines behave differently in practice.

The 2004 result that rewrote the diet sheet

For most of the twentieth century, gout advice was simply to avoid high-purine foods, and spinach, asparagus, mushrooms, cauliflower and dried legumes were on the list along with liver and sardines. Then a large prospective cohort of men, published in the New England Journal of Medicine in 2004 by Hyon Choi and colleagues, actually tested which foods predicted new gout.

Meat and seafood intake predicted gout, as expected. Dairy intake predicted less gout. Total protein intake predicted nothing. And purine-rich vegetables — the spinach and the mushrooms and the peas — predicted nothing either. That last finding has been repeated since, and it removed a whole column from the diet sheet: purine content in a vegetable does not translate into gout risk the way it does in a sardine.

Later work from the same research programme added two more levers with better evidence than most of the old list: sugar-sweetened drinks and fructose raise urate, and alcohol raises it in a graded way, with beer the worst — it contributes both alcohol and yeast-derived purines — spirits next, and wine much less associated in these cohorts.

How much diet can actually move the number

The honest answer is: something, but not usually enough on its own. A strict low-purine diet shifts serum urate by a modest fraction, and for someone whose urate needs to come down substantially to stop attacks and dissolve deposits, that fraction does not close the gap. This is why the 2020 American College of Rheumatology gout guideline puts urate-lowering therapy at the centre of management for people who meet the criteria for it, and lists dietary measures — limiting alcohol, purines and high-fructose corn syrup — as conditional recommendations alongside, not instead.

What diet and lifestyle do reliably deliver is everything else: weight, blood pressure, insulin resistance, kidney function, and the diuretics and other medicines that raise urate as a side effect. Gout travels with metabolic syndrome, and addressing that company is worth more than a purine table.

Water is the underrated part. Adequate fluid supports urate excretion, and for people who form uric-acid kidney stones — a related but distinct problem driven by persistently acid urine — urine volume and urine pH are the levers a clinician works with.

Разговор с тем, кто реально может проверить

Nothing on this surface is personal medical advice. If a food genuinely makes you ill, if you have had a kidney stone or a gout attack, or if you are considering removing a food group from your diet, the person to talk to is your doctor, pharmacist or a registered dietitian — they can test for the things that can be tested for, which a table cannot.

Частые вопросы

Do I have to avoid spinach and mushrooms with gout?

The prospective cohort evidence says no. Purine-rich vegetables did not predict new gout in the studies that looked, and they have been dropped from modern guidance. Meat, seafood, beer and sugar-sweetened drinks are the entries that survived.

How much can diet lower my uric acid?

Less than most people hope. Diet contributes about a third of the urate pool, and a strict low-purine diet moves serum urate by a modest fraction — usually not enough on its own for someone who needs a substantial reduction. That is why guidelines put medication first for those people.

Is beer worse than other alcohol?

In the cohort studies, yes. Beer carries both alcohol and yeast-derived purines, spirits carry the alcohol, and wine was much less strongly associated. All alcohol raises urate to some degree.

Does dairy help?

Dairy intake was associated with lower gout risk, not just neutral. Milk proteins appear to increase urate excretion. It is one of the few positive entries on the whole list.

Are cherries a treatment?

Cherry intake has been associated with fewer attacks in observational and small interventional work, which is interesting but well short of a treatment. It is not a substitute for urate-lowering therapy in someone who needs it.
