It looks exactly like an allergic reaction and is not one — there is no immune sensitisation involved, which is why someone who reacted to one tuna steak can eat tuna again safely. It responds to antihistamines. It is also one of the most commonly reported forms of fish-related food poisoning.
The critical point for a kitchen: histamine is heat-stable. Cooking, canning, smoking and freezing do not destroy it. Once the fish has made histamine, it stays made. The fish will usually look and smell normal, though a genuinely peppery or metallic first bite is a reason to stop eating.
What the regulations actually set
Because histamine in fish is a real and measurable hazard, it is one of the few compounds on this whole surface with a legal limit. EU food-safety criteria for fishery products from species associated with a high amount of histidine set the sampling plan and the limits directly, in the range of 100 to 200 mg per kilogram across the units tested, with a higher band for products that have undergone enzyme maturation treatment in brine. In the United States the FDA’s hazard-control guidance for fish and fishery products works with a defect action level for histamine in tuna and mahi-mahi that is lower still, because histamine is unevenly distributed through a fish and a low average implies a high spot somewhere.
There is no equivalent limit for cheese, wine or sauerkraut, which is one reason no reliable “histamine content” table exists for them. The figure varies with strain, temperature, ripening time and batch far more than it varies between products.
Histamine intolerance: what is known and what is claimed
The proposed mechanism is a shortfall of diamine oxidase, the enzyme in the gut wall that breaks down ingested histamine, so that a normal dietary load produces symptoms. It is a plausible mechanism. What is missing is a way to confirm it in an individual: serum DAO measurements, histamine-release tests and hair or IgG panels are not validated diagnostics, and the symptom picture overlaps heavily with IBS, migraine, mast-cell disorders, medication effects and genuine allergy.
That is why the guidance from allergy and dietetic bodies is consistent: exclude the things that can be tested for — IgE allergy, coeliac disease, mastocytosis where suspected — then run a short, structured low-histamine period of a few weeks under supervision, then reintroduce systematically. If symptoms do not improve during the restriction, the restriction is not the answer and should stop.
The reason supervision matters is that low-histamine lists are unusually destructive to a diet. Between them they remove most fermented foods, most aged protein, leftovers, many vegetables and most alcohol. Held indefinitely on a self-diagnosis, that is a genuine nutritional risk and a social one.
The practical rules that actually reduce histamine
Buy fish that has been continuously chilled, and get it into your own fridge quickly. Cook it or freeze it the day you buy it.
Cool cooked food fast and refrigerate it promptly; do not leave protein at room temperature to cool for hours. Reheating does not undo histamine formation.
Eat leftovers within a day or two, or freeze them on the day they were cooked rather than after three days in the fridge.
Where you already know a fermented food is a trigger for you, the fresher and less ripened version is usually the milder one — a young cheese rather than a long-matured one.
| Category | What is going on | Exemplos |
|---|---|---|
| Fish that produces histamine when mishandled | High free histidine in the muscle. Well-handled, cold-chain fish is fine; the same fish left warm is the classic scombroid hazard. | Tuna, Mackerel, Sardines, European anchovy, canned, Atlantic herring, pickled |
| Aged and fermented foods, where histamine is expected | Deliberate microbial activity over weeks or months. The amount depends on the culture, the time and the temperature, so no table can give you a number for your piece. | Cheddar cheese, Parmesan, Blue cheese, Gouda cheese, Salami, Sauerkraut, canned, Soy sauce, Miso, Natto, Red wine, Beer |
| Usually low when fresh and kept cold | Little histidine, little microbial exposure, or both. These are the staples a short elimination period is normally built on. | Chicken breast, Egg (hard-boiled), White rice, Potato (boiled), Carrot, Apple, Whole milk, Olive oil |
| Listed as “histamine liberators”, on weak evidence | These appear on almost every intolerance list on the claim that they release the body’s own histamine. The supporting evidence in humans is thin, and they are worth reintroducing early rather than avoiding by default. | Strawberries, Tomato, Cocoa powder, Orange |
O que muda o número
- Temperature and time are the whole story: the same fish is safe cold and hazardous after hours in the warm.
- Cooking, canning, smoking and freezing do not destroy histamine that has already formed.
- Ripening time drives it in cheese and cured meat — younger is generally lower.
- Alcohol and some medicines are reported to inhibit the enzyme that clears ingested histamine, which is one hypothesis for why wine features so heavily in symptom reports.
- Leftovers accumulate it. A dish is not equivalent to itself three days later.
Nothing on this surface is personal medical advice. If a food genuinely makes you ill, if you have had a kidney stone or a gout attack, or if you are considering removing a food group from your diet, the person to talk to is your doctor, pharmacist or a registered dietitian — they can test for the things that can be tested for, which a table cannot.

Perguntas frequentes
Can I cook the histamine out of fish?
No. Histamine is heat-stable, so cooking, canning and smoking leave it intact. The only control is keeping the fish cold enough, early enough, that bacteria never make it.
Is scombroid poisoning an allergy?
No. There is no immune sensitisation — it is a chemical dose of histamine in the food. That is why it can happen the first time, to anyone, and why the same person can eat that fish safely afterwards.
Is there a blood test for histamine intolerance?
Not a validated one. Serum diamine oxidase levels, histamine-release panels and IgG food panels are not accepted as diagnostics. Assessment is clinical: rule out what can be tested for, then a supervised elimination and reintroduction.
How long should a low-histamine diet last?
The restriction phase is short — a few weeks is typical — precisely because it removes so much of a normal diet. If symptoms have not improved by then, the diet is not the explanation and should be stopped rather than tightened.
Why do the food lists disagree with each other so much?
Because they are trying to tabulate something that is not a fixed property of the food. Two blocks of the same cheese, ripened differently, genuinely differ; so the lists are really lists of risk categories dressed up as measurements.
